Services Available

Clapp’s Nursing Center is a 118 bed skilled nursing center. All beds are certified to participate in the Medicare program with a limited number participating in the Medicaid program. A team concept, employing professional nurses, aides and other disciplines is used to deliver a quality resident care management system. Nursing services operate on a 24-hour basis.

We offer physical, occupational and speech therapy services in accordance with physician’s orders. Physical therapy services include gait training, exercises, heat and water massage, etc. These services are provided by a licensed physical therapist. Speech therapy concentrates on the treatment of communicating or swallowing disorders and is provided by a licensed speech therapist. Occupational therapy is a program or restorative retraining of the use of extremities through the practice of activities of daily living. A licensed occupational therapist provides these services on site through Heritage Healthcare Rehabilitation Services.

Each resident admitted to the facility must have an attending physician at all times. If the resident does not have an attending physician willing to provide services at the facility, a facility physician will be provided. The physician will examine the resident with 48 hours after admission and complete all necessary medical record forms. The physician will be responsible for writing admission orders, prescribing treatments and medications, and preforming routine follow-up medical care. The physician will visit the resident according to state and federal regulations and as needed. An attending physician or their designee is on call 24 hours per day. The Medical Directors of the facility monitor all physician services.

Our facility has an agreement with local hospitals for acute medical care. The responsible party or legal representative will be notified immediately if the physician orders the resident to be transferred to the hospital.

Our residents are provided therapeutic diets as prescribed by their attending physician. Balanced meals are provided three times per day with snacks offered between meals. Our dietary services are provided under the supervision of a Food Service Supervisor with consultation from a registered dietician.

Activities are designed to give our residents entertainment, socialization, exercise, relaxation and the opportunity to express creative talents and fulfill basic psychological, social and spiritual needs. In addition, we have partnered with local churches and community volunteers to supplement and enhance the activity and companionship needs of our residents. A monthly calendar is placed in each of our residents’ room to keep them up to date on upcoming events.

Our fantastic volunteers are invaluable in assisting with and enhancing our activities programs. Volunteer packets are available in the front office if you or someone you know would be interested in donating your time and talents to our wonderful residents.

Religious services are held in our facility at least once per week. We strive to meet the religious needs of our residents and therefore a variety of religious services are presented by local church groups and other volunteers. Members of the clergy are encouraged to visit with residents and families as often as desired.

Our clinical social workers excel in coordinating the social needs of our residents. Services provided by our social workers include helping our residents make optimal use of the services available to them, assisting with referrals for residents needing community services, counseling families on emotional, financial and social issues, and planning and coordinating discharges. Our social workers act as liaisons between residents, their families, physicians and our facility.

(Housekeeping, maintenance and laundry) Our residents are provided with a clean, safe and protective environment, both internally and externally. These services are maintained in accordance with state and local codes and regulations. Laundry services are covered under the Medicaid per diem rate.

Our facility uses the services of Southern Pharmacy. All routine and emergency medications are available on a timely basis. Our Consulting Pharmacist makes routine visits to review medication regimens for our residents, provides staff education and is accessible to residents and their families. The pharmacist is an active participant on our Medication Advisory Committee, as well as our Quality Assurance Committee.

Provided weekly by licensed cosmetologists (contracted by the facility) on a fee for services basis.

Our facility contracts with outside service providers for the following ancillary services:

Our business office handles resident billing, financial, reporting, purchasing, resident trust accounts, and all other financial functions. Our business office is open Monday through Friday from 8:30am to 5:00pm.


Clapp’s Nursing Center offers both private and semi-private accommodations. We have made every effort to create a warm and caring atmosphere for our residents that offers comfortable healthcare services while respecting privacy and each individual’s personal dignity. All rooms meet fire safety standards and are equipped with overhead sprinkler heads, intercom system for the nursing station, and call bells at the bedside and in the resident bathroom. All rooms have personal TVs with cable services included. All rooms have telephone jacks for telephone installation at the resident’s request and expense.

Our residents can enjoy comfortable visits with their guests and families in our Family Lounge near the front lobby.

We have two private dining rooms that may be reserved through our Social Services or Activities departments for private family parties.
